The number on your screen shares your area code and often your first six digits. It looks like a neighbour, a local clinic, a school. It is neither. This is neighbour spoofing, and it exists because familiarity raises pickup rates by a wide margin.
How caller ID became a suggestion
Caller ID was designed in an era when only trusted carriers could originate calls. The number you see is supplied by the originating side, and for most of telephony's history nothing verified it. Once anyone could originate calls over the internet for fractions of a cent, that unverified field became a free branding tool for whoever wanted it.
Why local numbers work so well

What STIR/SHAKEN actually fixes
Caller ID authentication frameworks let the originating carrier cryptographically attest that a caller is entitled to use the number it presents, and let the terminating carrier check that attestation. On many networks that is why some calls now arrive labelled as verified. It reduces spoofing on properly participating networks, but calls that enter through gateways with weak attestation still slip through, so treat verification as a helpful signal rather than a guarantee.
